This Pre-Registration is designed to be quick and easy, and completing the form should take you 10-15 minutes. It is recommended that you enter your online medical history as soon as you receive your personalized link. This link will be sent via text or email once your surgery is scheduled. After you complete your online history, the Pre-Admission nurses will be able to access this information so they can organize and document your medical history to prepare you for surgery.
